Support Coordinating Services
AB Nurses Support Services provides Support Connection, Coordination of Support, and Specialist Support Coordination services under the National Disability Insurance Scheme (NDIS).
We understand that navigating the NDIS system can be overwhelming, which is why our dedicated team is here to support and guide you every step of the way.
Level 1: Support Connection
- The lowest level of support coordination, providing assistance to build skills and understanding to navigate the NDIS.
- Focuses on helping individuals implement their approved NDIS plan and connect with service providers.
- Assists in resolving minor issues and supports individuals to monitor their plan and make adjustments as needed.
Level 2: Coordination of Supports
- A higher level of support coordination, suitable for individuals with more complex needs or circumstances.
- Provides more intensive support in implementing and managing the NDIS plan.
- Assists in coordinating services from multiple providers, addressing barriers, and resolving more significant issues.
Level 3: Specialist Support Coordination
- The highest level of support coordination, intended for individuals with very high or complex needs.
- Offers highly specialised support and guidance in managing complex service delivery arrangements and resolving significant challenges.
- Focuses on building skills and capacity to exercise choice and control over the NDIS plan and supports.
